Paul Sheldon (James Caan), a novelist recovering from a car crash, wakes in the remote home of Annie Wilkes (Kathy Bates), a nurse who claims to be his number-one fan. Annie has read every book he's written, and she's furious about the direction of his latest manuscript. As Paul realizes he cannot leave, and Annie's grip on reality splinters further, he understands that his survival depends on rewriting his story exactly as she demands.
